Description
POSITION PROFILE:
This position provides direct and indirect services to a caseload of culturally diverse individuals and/or families, including comprehensive assessment, information and referral, case planning, parenting education using evidence-based models, advocacy and other supportive services. This position is eligible for hybrid work schedule. Work hours typically fall Monday-Friday with the need for early morning, evening and weekend appointments based on client schedule.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
- Provide case management services to include interviewing, assessing, case planning, solution seeking, goal setting, referral, advocacy, and parenting education.
- Manage caseload of 15-18+ cases with an average 13-18 visits per week to include intake, client needs assessment, goal setting, develop individual service plans, parenting education, advocacy for client development, and follow-up services.
- Implement identified evidence-based parenting education models with fidelity (e.g. SafeCare and Systematic Training for Effective Parenting- STEP).
- Maintain accurate client records, document case notes in timely manner, and complete funder reports as requested.
- Schedule and conduct home or in-person visits with clients for intake and or client servicing appointments.
- Coordinate and conduct group facilitation may include trauma informed services, parenting education using the Systematic Training for Effective Parenting (STEP) curriculum, socio-emotional curriculum, or other topics of interest.
- Develop and maintain positive relationships with clients and other connected program partners.
- Participate in ongoing individual and group supervision to strengthen professional skills.
- Perform home-based visits as primary service location unless client needs/safety issues dictate otherwise.
- Perform visits for client sessions with flexibility and attention to client’s schedule (including early morning, evenings and weekends as needed).
- Perform outreach activities within the community as needed.
- Remain culturally sensitive and respectful in all client, staff and community interactions.
- Attend and participate in partner and funder-related meetings, trainings.
Requirements
QUALIFICATIONS (Education, Experience and Certifications):
- Bachelor’s Degree in Social Work, Child and Youth Development, Humans Services, or related field preferred
- Experience in case management, group facilitation, direct client service, and delivering curriculum-based interventions with youth and families.
- Demonstrated experience working with diverse populations and cultures and socio-economic families
- Bi-lingual English/Spanish preferred
- Experience working with school systems is highly desirable.
KNOWLEDGE, SKILLS AND ABILITIES:
- Ability to demonstrate knowledge of trauma-informed care and practices
- Solid proficiency of Microsoft Office to include Word, Excel and Outlook
- Accurate data entry of time sensitive information in fast paced environment
- Strong communication skills - ability to document client services in clear and concise manner
- Bilingual in English/Spanish or other common immigrant language is preferred.
- Ability to effectively manage multiple demands and time-sensitive priorities.
- Works well with a broad range of diverse, cultural and socio-economic populations and backgrounds.
- Possess an awareness of multi-cultural critical issues and community conditions facing children, youth, and families throughout San Diego
- Access to reliable transportation and willing to use it for work-related purposes.
