Job Title: Clinical Nurse - Navigator
Location: Methodist Hospital
Department: Cardiovascular Services
Position Status: Full-Time, Days Monday-Friday
Position Overview:
The Clinical Nurse - Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. Partners with patients and their primary physicians to develop customized care plans based on individual needs and preferences. Collaborates with physicians, nurses, health professionals, social workers, and other team members to ensure timely delivery of appropriate tests and treatments. Acts as an advocate for the patient, balancing care needs with financial considerations to ensure treatments are both effective and efficient.
Key Responsibilities:
- Coordinate and develop comprehensive care plans for assigned patients.
- Partner with patients, families, and healthcare providers to tailor care to individual needs.
- Collaborate with multidisciplinary teams to ensure timely and appropriate care delivery.
- Advocate for patients throughout their care journey, addressing needs and concerns.
- Ensure care delivery aligns with organizational policies, regulations, and best practices.
- Balance clinical needs with financial considerations to promote cost-effective care.
Qualifications:
- Associates Degree in Nursing (ASN).
- Bachelor of Nursing (BSN) preferred.
- Active RN license in Indiana or Nurse Licensure Compact (NLC).
- Graduate of a nationally accredited nursing program.
- 3-5 years of clinical experience preferred.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).
- Other advanced life support certifications may be required per unit/department policies.
- Specialty certifications may be required based on the department's provision of care.
- Knowledge of personal computers, word processing, and database software.
- Strong clinical work experience and knowledge of medical terminology.
- Understanding of medical record requirements, regulations, and policies.
