Mid = $51.91

Summary:
The Intake Services Coordinator coordinates the referral development and pre-admission patient assessment process. This role operates under the supervision of a licensed social worker or licensed professional counselor.
Responsibilities:
Responds to intake calls and completes admission paperwork. Reviews newly admitted patient accounts for eligibility and authorization daily.
Utilizes assessments tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tools under authorization/supervision of RN, LPC or LMSW.
Communicates with referral sources, including family members, legal system, psychiatrists, primary care physicians, and other treatment facilities. Addresses higher level of care referrals in a timely manner. Promotes positive and professional interaction with patients and their families, peers, clients, etc.
Facilitates group, family and individual sessions and completes all associated documentation. Schedules unit assessments, meetings, and appointments as appropriate. Maintains appropriate ethical and clinical boundaries with clients, family members, and staff members.
Prepares and disseminates monthly reports and completes required follow-up on activities on behalf of the Director. Prepares and submits all operational tasks/documentation (census report, PIORS) as required by the corporate offices. Regularly collects data to complete program's key statistical reports as well as to provide input and/or feedback to appropriate customers/suppliers and to improve efficiency.
Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships. Assists in submission of reports and requests for documents from various regulatory agencies. Maintains compliance with state and federal confidentiality laws.
Identifies areas needing improvement and utilizes organization's performance improvement process. Corrects issues identified and reports errors to Director. Participates as an active member of the interdisciplinary team. Appropriately represents the organization at public and outreach events.
Performs other duties as assigned.
REQUIREMENTS:
- REQUIRED: 3 years related experience in a similar position or industry.
- Bachelors degree in Nursing
- AZ RN license
- Fingerprint Clearance Card
