The Minimum Data Set (MDS Coordinator) is critical in health care and is responsible for the MDS of short-term stay patients in Transitional Care. This position is responsible for the assessment and documentation process included in the MDS required by the Centers for Medicare and Medicaid Services. The MDS Coordinator will monitor MDS coding for accuracy and completion, as well as Care Area Assessment (CAA) management and Care Planning. Additionally, this position is dedicated to expanding the integration of MDS critical thinking, awareness, and competency into the care plan process.
The MDS Coordinator must be a Registered Nurse (RN) responsible for accurately assessing, documenting, and coordinating care for residents in nursing care facilities. This role involves collecting patient data to create a comprehensive care plan and ensuring compliance with state and federal regulations, particularly for Medicare and Medicaid reimbursement purposes. The MDS Coordinator also works collaboratively with the supportive living services line to establish system delivery that supports VMRC's mission, vision, and values.

Key Responsibilities
A. Essential Functions - Operations and Administration
1. Participates in overseeing all facets of Complete Living and Transitional Care to adhere to industry regulations and VMRC's program philosophy of service and care.
2. Performs comprehensive assessments of residents using the MDS tool to gather accurate data related to physical, cognitive, emotional, and functional health status.
3. Ensures MDS assessments are completed promptly within the timeframe CMS requires, including entry, admission, annual, quarterly, significant change, optional state assessments, and discharges.
4. Completes other MDS assessments to maintain compliance with federal, state, and local standards.
5. Works with billing and finance department to ensure accurate coding and billing is based on the MDS assessments.
6. Participate in a “triple check” with the billing and finance department to ensure accurate claims are submitted.
7. Ensures that residents' medical records and provider documentation accurately reflect diagnosis codes related to the care provided.
8. Review resident immunization records for accuracy and completion.
9. Monitors regulations and industry standards changes to ensure ongoing compliance with CMS guidelines.
10. Please participate and help in the admission process as requested.
B. Essential Functions - Resident Care
1. Ensures the comprehensive resident care planning process is completed according to state, federal and local standards.
2. Ensures each resident’s MDS assessments are completed and submitted accurately and timely.
3. Communicate staff training needs to DON.
4. Participate in the review of resident medical records for proper documentation.
5. Champion resident-centered care.

1. Compliance with all VMRC and service line policies & guidelines conscientiously.

3. Maintains confidentiality of all resident care and protected health information—reports known or suspected incidents of unauthorized disclosure of confidential information to supervisor or designee.

5. Uses correct body mechanics to prevent injury to self, residents, and other staff.
6. Assumes responsibility for and actively participates in staff development (professional growth) and continuing education to keep skills and required certification(s) current.

8. Attends all required in-services and service line staff meetings.
9. Follows emergency and evacuation procedures.

1. Resident Assessment and Data Collection:
Complete the Minimum Data Set (MDS) assessments for residents by state and federal guidelines.
- Conduct thorough assessments covering resident health's physical, psychological, and psychosocial aspects.
- Collaborate with interdisciplinary team members (physicians, therapists, social workers, dietitians) to gather accurate data for assessments.
2. Care Planning:
- Develop individualized care plans based on MDS assessments, ensuring they address the resident’s needs and goals.
- Regularly review and update care plans to reflect changes in the resident’s condition or treatment.
- Participate in care plan meetings with residents and their families, discussing care strategies, goals, and outcomes.
3. Compliance and Documentation:
- Ensure all MDS assessments are completed and submitted on time per regulatory deadlines.
- Maintain thorough and accurate documentation, keeping records up to date for audits and reviews.
- Stay informed about changes in Medicare and Medicaid regulations, ensuring the facility’s compliance with these rules.
4. Medicare and Medicaid Reimbursement:
- Review MDS data to ensure accuracy for Medicare and Medicaid reimbursement claims.
- Monitor Resource Utilization Group (RUG) scores to ensure proper reimbursement levels based on patient needs.
- Work with billing and financial teams to verify that care aligns with documented assessments and reimbursement requirements.
5. Interdisciplinary Team Collaboration:
- Facilitate regular meetings with the interdisciplinary team to discuss resident care and ensure the coordination of services.
- Provide education and training to staff on MDS processes, documentation standards, and regulatory requirements.
- Act as a liaison between clinical staff, residents, families, and administrators to ensure continuity of care and regulatory compliance.
- Accepts on-call responsibilities for weekends and holidays
- Works as a nurse in any care area as needed to meet the needs of the patient/residents and when MDS schedules allow.
6. Quality Assurance and Improvement:
- Participate in quality improvement initiatives, analyzing MDS data to identify trends or areas for improvement.
- Ensure resident care aligns with evidence-based best practices and quality standards.
- Assist in audits and surveys conducted by regulatory agencies to ensure compliance with healthcare standards.
7. Resident and Family Communication:
- Communicate with residents and families regarding care assessments, care plans, and any changes in treatment.
- Educate residents and families on the care process, ensuring they understand the goals and outcomes of care planning.
