We’re recruiting full-time health coaches with the NBC-HWC credential for a digital health partner focused on remote patient monitoring for chronic condition populations. 

Overview:

• Conduct monthly 1:1 coaching calls for patients with chronic conditions (e.g., diabetes, hypertension, cardiovascular disease).
• Provide ongoing guidance and check-ins, using remote monitoring tools to track patient progress.

Requirements

Requirements:

• Must be a National Board Certified Health & Wellness Coach with the NBC-HWC Credential.
• If you've taken the NBC-HWC exam and are currently waiting on test results, you may apply.
• Expertise in educating patients on self-management strategies, disease management, medication adherence, symptom recognition and preventative care.
• Ability to work independently in a remote setting. Telehealth or remote patient care experience preferred.
• Must be located in the U.S.
