Who You Are:
We are seeking candidates who are customer service oriented and take great pride in serving others in a caring and professional manner. Must be a licensed Nurse Practitioner, and will be responsible for providing primary care medical services to Westminster residents in our Skilled Nursing, Rehab, and/or Assisted Living, as well as our Wellness Clinic .
ESSENTIAL FUNCTIONS:
Take proactive approach to healthcare by providing quality of care to all patients.
Provide direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment.
Order diagnostic tests, such as x-ray, CT/MRI, electrocardiogram, and laboratory tests, and interpret test results for deviation from normal.
Document findings and treatment course in patient chart including follow-up notes.
Instruct and counsel patients regarding compliance with prescribed therapeutic regimens. Communicate with patients regarding testing results, follow-up care, and additional information.
Communicate with Community Administrator, Director of Nursing, and/or Resident Health Services Director regarding resident health and wellness concerns or issues.
Maintains or improves independence through directing, educating, and orienting residents in the use of appropriate adaptive equipment or techniques. Recommends home assessments to improve safety at home.
Prepares and maintains required reports, records, studies and surveys.
Serves as a resource person for residents and assures that all necessary forms are prepared and informational pamphlets are distributed.
Be available and actively engaged in support of community needs in times of emergency situations and perform other duties as assigned.

Requirements
Education:
Must be a graduate from an accredited school of Nursing; must have a current license to practice as an APRN in the State of Texas.
Experience:
Should have at least three years of experience in geriatrics as Nurse Practitioner.
