Description
Provides safe, direct patient care to promote health and prevention to the population serviced. Educates, advises, and makes safe and appropriate decisions according to protocols and standards of care. Collaborate with medical providers and multidisciplinary team members.
Provides direct, professional nursing care to patients utilizing the nursing process of assessment, planning, implementation, and evaluation.
Delivers patient-centered care to patients and families by providing care coordination and care management services.
Performs in-house and phone triage to assess patient acuity and their needs for appropriate and timely access to care.
Educates individuals and families on recommended services and exams that are due across their lifespan.
Participates in quality improvement projects and initiatives within the medical department.
Complies with all of the instituted health division policies and procedures applicable to job position.
Ensures strict confidentiality of health records.
Provides care in a culturally sensitive manner to the population served.
Performs other duties which are delegated and required.

Requirements
Bachelors in Nursing preferred; Associates Degree considered
Valid Registered Nurse license for the state of Wisconsin and basic life support (BLS) is required
3 years’ experience in a patient care setting of patients across the lifespan is required
Ability to provide care in a practical manner according to training, experience and approved competencies
Experience with delegation of approved tasks
Knowledge and experience providing care coordination and care management services
Knowledge of health education theory and practice
Knowledge of chronic conditions and preventative care for patients across the lifespan
Knowledge of applicable federal, state, county and local laws, regulation and requirements
Experience working with electronic medical record systems
Valid Wisconsin driver’s license required; must maintain a valid Wisconsin driver’s license as a condition of employment
Must successfully pass all applicable background checks and drug screens
FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
